Teemwork.ai tax form error

I recently signed up to apply for a crowd-work job over on teemwork.ai (after some bumps in the road with another wfh site). Well, I'm at the point where I need to fill out my W-9 as part of setting things up, but on my desktop, when I go to fill in my SSN, it keeps showing this error:

I tried from my phone and while this particular error doesn't show up on mobile, I get to the end of what I need to fill in/sign, hit "Validate and Send", but then am told "Some fields still have to be completed!". Has anyone else recently tried to sign up with teemwork.ai and come across this issue? I've reported the copy/pasted one a couple of times and no luck yet. I can't tell what I'm doing wrong, as the fill-out guide only shows I have to fill in my SSN and signature/date. Any help would be greatly appreciated!
