20/20 Research runs various focus groups that always pay well. They also seem to offer surveys on their website as well. After signing up, they will email you about available studies, and they have a page to view available surveys. However, it does not appear to show all of the available studies. I recommend keeping tabs on their facebook page where they post most of their studies. After signing up, you will need to confirm your email within 8 hours.
The majority of the studies are online, but there may be some in-person ones as well. Despite being online, many studies require users to be in a certain state or city. Some tests will require a live interview with a webcam and/or phone, but others are unmoderated. The exact requirements will vary depending on what the researcher needs, so be sure to check the requirements when completing the screening questionnaire. I haven't personally used this site before, but people have reported being paid by it in one of the groups I'm part of.
Signup Requirements: First name, last name, email, password, phone number, address, date of birth, job title, and various other demographic questions. They appear to be primarily USA focused, but Canada is one of the available options for the country, and they have an "other" box too.
Available Rewards: Paypal, Amazon.com, iTunes, Best Buy, Fandango, Reward Link Preferred, Walmart
