Harmful content on Appen Yukon?

I've been going over the guidelines because tomorrow I start the exam and I keep seeing the mention of "harmful content". How often does that happen? It seems to be a stressed point so it can't be 0%. Whether it happens rarely or frequently, I'm still putting my all into passing the exam. I'm just curious.
