Tax forms I should be expecting to receive from ERewards?

Greetings. I have had an ERewards account for a few years and in 2020 I made a lot (approx. $1,400) in profits in actual USD from them. They made me mail in a tax form including my SSN and street address to continue taking surveys because I had gone over the 1 year threshold of earnings and would have to claim my earnings on taxes.

My question is, for those of you who have made large amounts of money with surveys, what tax form should I be expecting in the mail?
