You can also claim lost hours up to 5 hours($20 per hour Self-certification) and up to 20 with documentation and you may eligible for up to 10k out of pocket losses total. There are also multiple ways to get your payment for the $50. Check, E-Check, Paypal, Venmo, and Amazon Credit.

You got time to fill this out but: You must complete the form, either by mail or online by March 30th, 2020.
