Question for Canadians who use Upwork.

So I signed up for Upwork but freaked out when they asked for that Tax form agreement that's for income from the US or outside it. I'm a permanent resident and I can't take risks. Plus the fact that I'm a full-time student. How legit or risk-free is agreeing to that form and adding it to your yearly tax? How complicated would it make the situation?
