UHRS blocked HitApps reasons

Today I was blocked in an HitApp with 95% quality rating and I'm also sure that I have not exceed the time per hit limit. Since it's not the first time that this happen, are there any another reasons to get blocked? Is there a way to unblock these Hitapps? Thanks.
