Did anyone get their redemption denied by upv?

After redeeming my first time with a couple hiccup i.e emails back and forth with upvoice I manged to get my gift card, but the 2nd time I redeemed a gift card, well, I was greeted by "request has been declined", has anyone else experienced this? US region btw!
