QuickThoughts W9 help

Hi guys. I'm just trying to get a head start on filling out and mailing my W9 because Im pretty sure im getting close to the $599. I also don't want to wait a couple weeks to cash out. I have most of my paper filled out but I just forgot exactly where on the W9 I am supposed to put my login email. Was it at the top of the page or can u put it in the section right below where you put your address, that says "List account numbers here (optional). Thanks in advance
