Can't move on to complete my profile on i-say rewards...anybody else had the same problem?

I'm stuck at 59% where it asked me "Which of the following conditions, if any, have you experienced in the past 12 months" but no conditions are shown. I delete and redownloaded the app but that doesn't work. Seem like it's preventing me from getting surveys as i haven't received any for a little over a week when for the first couple of days i was getting quite a bit.
