Problems with SurveyJunkie?

Is anyone else having problems cashing out with SurveyJunkie?

1. The PayPal and gift card options are not available to me.
2. I can't cash out via bank transfer; every time I try it says it sent me the verification number (but nothing appears in my email).

I've checked my settings and email multiple times. I've also tried contacting support (a month ago)... they said they'd be in contact in about a week and that was a lie. They never called me, and they told me to check for some popup on my account page (there was none).
